Hypospadias in Children: Understanding, Treating, and Thriving

By Dr. Dhruv Mahajan; Pediatric Surgeon, and Pediatric Urologist

As a pediatric urologist, one of the common conditions I encounter is hypospadias. It's a condition that can be alarming for new parents, but understanding it better can help you navigate the journey more confidently. This guide aims to provide you with essential information about hypospadias, its causes, treatment options, and what to expect.

What is Hypospadias?

Hypospadias is a congenital condition affecting the urethra, the tube that carries urine from the bladder to the outside of the body. In boys with hypospadias, the opening of the urethra (meatus) is located on the underside of the penis rather than at the tip. The severity of hypospadias can vary:

- Distal Hypospadias: The opening is near the head of the penis.

- Midshaft Hypospadias: The opening is along the shaft of the penis.

- Proximal Hypospadias: The opening is near the base of the penis or in the scrotum.

Causes of Hypospadias

Hypospadias occurs during fetal development, typically in the first trimester of pregnancy. The exact cause is not always clear, but several factors may contribute, including:

- Genetics: A family history of hypospadias can increase the likelihood of a child being born with the condition.

- Hormonal Factors: Disruptions in the balance of hormones during pregnancy can affect the development of the urethra.

- Environmental Factors: Exposure to certain substances or medications during pregnancy may play a role.

Diagnosis

Hypospadias is usually diagnosed at birth during a physical examination. In some cases, the condition may be detected during prenatal ultrasounds. If hypospadias is suspected, your pediatrician may refer you to a pediatric urologist.

Treatment Options

The primary treatment for hypospadias is surgery, typically performed when the child is between 6 to 18 months old. The goal of surgery is to:

1. Correct the Position of the Urethral Opening: Move the urethral opening to the tip of the penis.

2. Straighten the Penis (if necessary): Correct any curvature of the penis (chordee) to ensure proper function.

3. Improve the Cosmetic Appearance: Ensure the penis looks as natural as possible.

The specific surgical approach depends on the severity of the hypospadias. Surgery is generally successful, with most boys achieving normal urinary and reproductive function.

What to Expect After Surgery

After surgery, your child may need to stay in the hospital temporarily. Pain management and care for the surgical site are important during the recovery process. Follow-up visits will be necessary to monitor healing and ensure there are no complications.

Long-Term Outlook

Most boys who undergo hypospadias repair go on to lead normal, healthy lives. The long-term outlook is excellent, with successful restoration of function and appearance in the majority of cases. Maintaining regular check-ups with your pediatric urologist is important to monitor your child’s progress.
